This file contains a copy of a pilot questionnaire compiled for Malcolm Johnson's 'Ageing, Needs and Nutrition Study', conducted at the Nuffield Centre for Health Services Studies, University of Leeds. The questionnaire focusses on childhood experiences under the general questions, What kind of a home were you born into? and What kind of a life did you have as a child?
