"A continuacion of the chronicle of England", a prose compilation by Richard Grafton, has a separate titlepage with foliation commencing on Aa2.
Leeds University Library copy BC GB C16/17 HAR has the title details of STC 12766.7. It also has the continuation of Henry VIII's reign at the end in 14 folios, missing only CLX and verso with colophon.
