'The lives, actions, and ends of certain notorious hereticks' has separate pagination and separate title-page, with title: Apocalypsis: or, the revelation of certain notorious advancers of heresie... faithfully and impartially translated out of the Latine by J.D. 1655. The Epistle dedicatory is signed: Jo. Davies.
Though Wing(2) catalogues the two parts separately, RLIN and OCLC state that they are part of the same work, and the signatures, though not continuous, suggest that they were issued together (Part 2 begins at signature Aaa, rather than starting again at A).
J.D. (translator of Apocalypsis) = John Davies (RLIN). "Apocalypsis..." is sometimes partially attributed to Henrick van Haestens (RLIN).
